Attaché at NinthFlr is not liable for the acts, errors, omissions, representations, warranties, breaches, or negligence of these suppliers, or any personal injuries, death, property damage, or other damages or expenses resulting therefrom. Attaché at NinthFlr has no liability and is unable to make any refund in the event of any delay, cancellation, overbooking, strike, travel restrictions, force majeure, or other causes beyond its direct control. The terms and conditions of the vendors and suppliers govern the purchased services or product and we urge you to carefully review all applicable cancellation and refund terms. Furthermore, conditions can change rapidly in a country at any time. It is your responsibility to check the USA Government travel advisories for your intended destination at travel.state.gov/content/travel.html.
PASSPORTS & VISAS
U.S. citizens traveling to any destination outside of the United States will be required to present a valid U.S. passport. Passports must be valid for at least 6 months past the return date. It is recommended to have at least two blank pages in your passport. Some countries require a visa for transit or entry. All names on travel documents must match the legal name of your passport or government-issued photo identification. Please check the State Department website at https://travel.state.gov for further details. If you are a citizen of another country, you may have additional requirements and should check with the nearest consulate or embassy of the destination you are traveling to for more details on entry requirements for non-U.S. citizens.
I have read and understood the passport, visa, and validity requirements for the destination(s) that I am visiting. I am aware that I am responsible for having the appropriate documentation regardless of wait times or service errors by the U.S. Department of State and passport agency. I understand that failure to comply with these regulations may result in denied boarding, denied entry, and/or government-imposed fines which will be my sole responsibility.
There is an inherent risk in contracting COVID-19 and its variants while traveling during the pandemic and each traveler must assess their level of risk tolerance. Travel restrictions and requirements (including required documents, proof of vaccination, testing requirements, and mandated quarantine) are constantly changing, including after the commencement of travel. We strongly urge you to check the latest guidance from the CDC and the State Department travel advisories and review the individual supplier’s websites before traveling.
I have read and understood the current COVID-19 pandemic-related entry, testing requirements, and travel warnings for the destination(s) that I am visiting, as well as the testing requirements for re-entry into the United States. I will comply with the applicable government laws and regulations of the destination I am visiting. I also understand that entry and testing requirements may change before, during and prior to my return to the United States. In addition, I understand that if I test positive for COVID-19, I may need to quarantine in designated facilities for the specified quarantine period in effect and will need to wait until I test negative to be allowed entry back to the United States. Any additional costs and expenses (including testing, quarantine stays, and rebooking fees) are my sole responsibility.
For your protection, we strongly recommend purchasing travel insurance and some destinations have this as a COVID-19 requirement. Travel insurance provides cancellation and interruption coverage as well as emergency medical expenses, emergency medical transportation, lost or damaged baggage, trip delay, and more.
I accept the responsibility to read and familiarize myself with the details of the travel insurance policy information provided by my travel advisor, and that questions regarding coverage should be directed to the insurance company. Any insurance claims that may fall under the relevant travel insurance policy must be submitted to the insurance company directly.
